Insurance:
Our practice participates with most HMO and managed care plans. We also accept Medicare assignment. Please be sure to obtain a physicians' referral prior to your visit. Not having a valid referral on file on the day of your appointment may delay your appointment to another date. Copayments are collected at the time of your visit. |
Appointments:
The practice is open Monday through Friday by appointment only. When calling to schedule an appointment, please state the purpose of the appointment clearly to allow for the appropriate amount of time to be scheduled. To schedule appointments, please call during business hours.
Please be considerate to fellow patients and when necessary cancel your appointment 24 hours in advance. There is a $25.00, non-refundable, fee for No Shows as well as appointments cancelled within 24 hours.
The phone lines are open from 8:00am through 5:00pm on weekdays and our reception staff handles all of our calls. If you need to speak directly to a doctor, one will return your call when time permits. If you are having an emergency, please call 911.
